In the receipt: the name of the beneficiary of the certificate, the name of the institution, the surname of the cashier of the institution receiving the receipt, the date (... December 1947). In the control coupon: the surname, name, patronymic of the recipient of the certificate, sex, year of birth, residence permit, signature of the management board. On the reference series: "P". The inquiry is not completed, there is a round stamp and the signature of the head of the enterprise
